The Public Option: Five Reasons Harry Reid Went For It

Less than two weeks ago, Senate Majority Leader Harry Reid's aides and colleagues were saying they were all but certain he would bring health care legislation to the Senate floor without a public option. Yesterday, as everyone who follows this issue now knows, he did the opposite. It's a big gamble. While Reid will likely be able to hold his caucus together and get all 60 votes he needs for the procedural vote to bring the bill to the floor (known as a "motion to proceed"), he's still at least three votes short on the more crucial vote of cloture--that is, cutting off a promised filibuster by Republicans.

So why did he go for it? There are some subtle shifts in the politics around public option on Capitol Hill that have given it a new--though still fragile--lease on life. Among them are these five:

1. The poll numbers. As Swampland readers know, the public option has enjoyed strong public support all year. But what surprised some on Capitol Hill was that it continued to do well in a more recent round of polls, even after the craziness of the August town hall meetings. What that says to politicians is that the sentiment for a public option is out there, and it's resilient.

2. The CBO numbers. Another boost came from the Congressional Budget Office, which showed that a public option could save money, and that a strong public option could save even more. That puts conservatives in the unsettling position of standing for more spending.

3. The insurance lobby overplayed its hand. They had assumed that a consultants report warning of higher premiums under health reform would deal a crippling blow. But the methodology was so skewed that its main effect was to damage the insurers' own credibility -- and to diminish their goodwill on Capitol Hill. The argument that a public option might blow a hole in the insurance industry's business model isn't getting as much sympathy as it was a few weeks ago, when the insurers were considered a crucial ally to keep at the table.

4. More horror stories. Recent weeks have seen a spate of reports showing how badly insurance companies can act when they are left unchecked: one baby denied coverage because he was too big, another because she was too small, and a woman who couldn't get health insurance because she had been raped and was taking anti-HIV drugs as a preventive measure.

5. Home-state politics. Specifically, Harry Reid's poll numbers back in Nevada, where he faces a potentially difficult re-election fight next year. If Reid is to survive, he can't afford a disaffected Democratic base. Win or lose on this one, he will at least be able to tell liberals he gave his best effort to a cause that has, for them, become the most important element of health care reform.

    6. The smoke and mirrors that Obama has been giving this issue has been unprecedented. Flat out lies to the public on a so-called public option, and how much money will be saved due to all of the arm twisting of the CBO.

    7. Seniors have not been truly "whipped" into a voting block as of yet. The real money is still yet to be spent on making it known to Seniors how much of their Medicare benefit will be lost when this legislation is passed, if it gets that far.

    8. Media darlings like Karen Tumulty, who constantly press for health care reform, and simply lie about what the true costs will eventually be. That the so-called "savings" are really more smoke and mirror tactics by Obama and his henchmen. The tax payers of the US will start paying for this health care benefit now, but will not see any benefits until at least 2013.

    9. Polls do show most Americans want health care reform, but not THIS reform package. Americans want to see less costly and expensive reform passed first. They want legislation to stop the practice of pre-existing conditions. They want TORT reform. They want an investigation and abuse of Medicare stopped. All of these measures do not cost anything.

      I'm skeptical. Since when does a government program reduce costs? And what did the CBO say originally about cap and trade? As I recall, JNS was touting the CBO's scoring which turned out to be, um, less than right.

      And the CBO's scoring really isn't an answer to my question, since the CBO is not (a) taking account of changes to the program which will inevitably result and (b) dealing with unintended consequences of the public option.

      In other words, the CBO isn't the whole story, and it's portrayed as such. That's wrong. The American people are being asked to allow Dems to rework what, 1/7th of our economy, and the say-so of the CBO on costs is unchallengeable.

      The absence of a government role in administering health care is why it is 1/7th (1/6th is the number I have usually heard)of the economy,while it is about 1/10th in the rest of the OECD. And better.

      There are plenty of instances where government lowers costs.Or, where outsourcing, as with Medicare Advantage makes things more expensive.

      Whether and how much it will save depends on the specifics of the bill, but in general, the answer to your question is a resounding yes.

      The empirical evidence from every single other form of publicly run insurance shows lower costs and higher service levels vs. the private system we have now.

      For an example besides all of the other countries everyone likes to point to, check the U.S. military and TriCare. When I signed up for TriCare Reserve Select, it took ten minutes and I had two options: $48/month for single coverage, $251/month for self and dependent coverage.

      Plus, do we even want to lower costs? My guess is that we do, but we also want to make sure what costs are being lowered and how. Tort reform strikes me as a good idea. But one of the great things about our health care system is the innovation. I wouldn't want to kill that.

      Okay, shepherd, I really have to take issue with your life-expectancy comment. The main reason why life expectancy is lower in America than some other industrialized countries is because of: lifestyle choices. Americans simply over eat (hello obesity!), do not exercise enough, and more often live a sedentary lifestyle. What's the number one cause of death in America? Heart disease! Also, many older Americans used to/do smoke heavily (I say older Americans since the numbers with younger people and overall have dropped since the 80s).

      Lifestyle choices and biology also contribute highly to our high infant mortality rate.

      Not sure if anyone has studied this, if so feedback would be appreciated, how are the survival rates of diseases such as cancer in America compared to other countries? That is, not the amount of deaths (since this is heavily influenced by incidence) but the expectancy once someone is diagnosed?

      That's the real reason the public option is in there. If the bill has an individual mandate, which it must, it will need a satisfactorily effective public option. Without one, good bye liberal base, good bye Blue Dogs, good bye Harry, so long Democratic majorities of the future. Harry has no choice. Only conservadems who have been offered a better opportunity than their current Senate jobs (or who have nothing to lose anyway, Mr. Lieberman) will kill it at this point.

    Isn't that whistling past the graveyard? Let's just assume that costs would go done, and that's real costs, not "projections" from the CBO. They still gotta sell it. The public believes costs go up, and conservatives can still make that case. It's not like the CBO's numbers are sacrosanct.

They couldn't wait to tout their numbers when it came out over a trillion dollars, even though you were willing to focus on partial reports and unrevised figures. But of course now that the numbers are not in your favor, in fact show up your lies for the lies the rest of us always knew they were, the CBO has the credibility of some old hack scribbling on the back of a napkin. Let's face it, conservatives are absent anything even close to resembling integrity. They want to win regardless of how shameless they have to be in order to do so. They touted CBO as a non-partisan, unassailable research -- but now that they don't like the numbers they want to destroy the representation of the in favor of what that faulty industry report that got laughed out of the room?

They are likely a pretty good floor for what something is going to cost. (Jeez, are you really that stupid?) Latest stats show 92% of Canadians love their universal health care and wouldn't give it up for anything.

      It's hard for every American to find an opportunity to be successful when far too many of them are held back by ill health or the bankruptcy of their family due to a family member being ill. By improving the stock of everybody in the country, and giving everyone a fair and equal opportunity, your country as a whole could be raised and it would no longer be like a third world country of haves and have nots. It's also the morally right thing to do. You finally have a president who understands moral right and wrong. Now it's up to the citizens to follow his example.

    Universal coverage and the public option are very appealing but every government study shows that premiums and costs would not go down but would instead increase. There is not one government program in existence that has not increased ten to more than one hundred times the cost the government initially claimed it would cost. Food stamps have increased by 10,000 (yes, ten thousand) percent from what they were initially claimed to cost the government, and no sane person believes the number of poor people in this country has increased by 10,000 percent.

    All those who criticize people who are worried about the costs... beware! We will be paying more money than we are now, and the only way the government will be able to keep costs down is to ration care--which means that certain medical tests you can get now, you will not be able to with a public option. If you think that's fine, then great. Let's just hope that until you die at a ripe old age you are never, ever sick because under a government plan certain tests will be denied to you. And for the elderly, all I can say is good luck! Obama wants to save $800 billion over ten years by cutting Medicare costs. It's an absurdity that digitizing medical records will save that many billions. If you truly believe you can cut out $800 billion dollars from Medicare without cutting benefits and rationing care, then I have a bridge in Brooklyn I would like to sell you.

    Unless and until the government is going to seriously change the way the insurance companies, trial lawyers and pharmaceutical companies do business, any system will fail. John McCain stated during the campaign that the Republicans are in bed with the insurance companies, the Democrats are in bed with the trial lawyers and both parties are in bed with the pharmaceutical companies. He was 100 percent correct--we can bandy about terms like "public option" all we want, but until we rein in the the above three groups that are bankrupting the health care system, nothing much will change. Unless we change the above, a government plan will not save money and will end up severely rationing care for everyone.

    True presidential leadership involves going after the insurance companies, trial lawyers and drug companies, and doing what's best for the American people, not one political party. Rhetoric will not pay for a good healthcare system, nor will it correct the problems that are putting more and more Americans on the edge of bankruptcy and despair due to rising healthcare costs and denials of coverage.

      This is an excellent post, except you forgot the most important player in the increase of medical costs: Doctors and Hospitals. After all, they are the place where the medical bills are first generated.

      Until you deal with how doctors do business, no "reform" will reduce medical costs. Right now we pay "fee for services" instead of "fee for results". Until we change this system for fees, medical costs will continue to rise.

      The price of medicine is rising because the doctor's fees are rising. It is not all the doctor's fault. Medical school loans, malpractice premiums, paperwork, you name it, it is much more difficult to practice medicine than it was 30 years ago. At the same time, a number of doctors do tests and charge fees for unnecessary procedures which drive up the medical bill. If we can stop or limit that practice, that alone would safe millions if not billions without reducing the quality of care.

      It is easy to blame the insurance industry. But HMOs were designed to keep the costs of medical care down. They are businesses, not non-profits and as a business, they are in the business to make money.

      It is easy to blame big "Phama", especially since they have the most lobbyists fighting this battle. But they are a business as well, as it takes years to develop a drug and bring it to the marketplace.

      It is easy to blame lawyers. First of all, for full disclosure, I am an attorney, though I generally avoid medical malpractice. Blaming the trial lawyers is like blaming guns for crime. I know conservative like to pick on trial lawyers, but when you look at malpractice claims from 1997 to 2006, they are down 4%. Total tort claims are down 21%. Yet malpractice premiums continue to skyrocket for doctors. That is because malpractice premiums are not tied to claims or even the amount of awards, they are tied to the carrier profits. Insurance companies took a big hit in the recent market crashes and needed a way to make up the money. What better way than mal practice premiums, because they have the doctors by the cojones.

      If you want to reduce what doctors pay out in malpractic claims, I suggest you look at what the University of Michigan did. It would take too long to spell out all they did to reduce claims, but between 2002 to the present, the University of Michigan reduced the money set aside for malpractice payoffs from $700 million a year to just $13 million a year.

    Also, for every horror story you hear about healthcare here, there is a horror story about healthcare in countries with a government-run system. I just don't mention them because, again, I don't think it's wise to base a decision of this magnitude off of anecdotes.

As far as the canadien thing, if you can reference what you have or have not heard from them, so can I. Blind robotic follower? What a hoot! I follow nobody except my own intellect. Yet, you are attacking me for my beliefs and making VAST leaps to conclusions about me. Yet, I am the irrational one. If that's not the pot calling the kettle black, I don't know what it. You can choose to try and render my remarks moot by leaping to the worst possible conclusions about me, but you would be very wrong and engaging in the sort of intollerance I thought Libs abhored. You are dangerous because of what you believe. "for the common good". Will "from each according to his ability to each according to his own" be the next great socialist/communist term to flow from your oh so enlightened and tolerant lips? Grow up. Other than saying you are dangerous (which I firmly believe your beliefs to be), I made no attack or assumptions about you, yet you attack and make all the worst assumptions about me you can. It may help you to belief the opposition is all wack-o, but you do so at your own peril. I'm sure your alleged military service has taught you the dangers of underestimation the opponents.     I'd also hazard a guess many of the people criticizing Lieberman were also criticizing Republicans for calling Snowe and the like RINO's. Liberal mentality, it's only bad if you do it. When we do it, it's just "speaking truth to power" or some other inane phrase we've come up with to snow the uninformed.

    The administration should propose to try to administer a health care trial in a state or two. It should use the data on real costs of the trial for rate setting. If the government can prove they can lower costs and provide better care, who could oppose a public option. The insurance industry should be able to monitor and audit the government trials. I doubt the government could administer anything well, but let them show they can in a state or two; Georgia and Kansas maybe.

    Still unanswered is how many doctors and dentists will participate. None of the dentists in my local area will even participate with Delta Dental and many doctors won't participate with Blue Cross. How is the government going to get health care provider participation? Insurance that no local provider, or only a few poorly thought of providers, is no insurance for the poor.

    Don't see any real cost containment. Politicians don't contain costs, they protect rich donors and that runs up costs everywhere. Not a bad idea; but like utopia I doubt the government can do it well.
